| Those moderate to severe frown lines between your brows form over time as the result of muscle activity. BOTOX® Cosmetic is injected directly into the muscles between your brows and around your eyes. It works by blocking the transmission of nerve impulses to the injected muscles; this reduces the activity of the muscles that cause those lines to form. |

| When will I see results from a BOTOX® Cosmetic treatment? |

| After a few days, you may see a marked improvement in the moderate to severe lines in the treated areas. Lines continue to improve for up to a month, and results can last for up to 4 months. In clinical trials, nearly 90% of men and women rated the improvement in their appearance as moderate to better 1 month after treatment. Results may vary. |

| What is botulinum toxin A? |

| It is a toxin produced by the bacteria Clostridium botulinum which temporarily blocks the action of muscles. |

| Is botulinum toxin A safe? |
|
| Botulinum toxin is an approved treatment for Blepharospasm, hemifacial spasm and facial wrinkles. Long-term follow-up studies have shown it to be a very safe and effective treatment with up to 90 percent of patients obtaining almost complete relief of their Blepharospasm. |

| How is botulinum toxin A used for therapeutic uses? |

| It is used to treat Blepharospasm. Blepharospasm refers to eyelid muscles around the eye that close involuntarily. This may cause loss of vision, especially while reading, headaches and eyebrow strain. The early symptoms of blepharospasm include increased blink rate, eyelid spasms, eye irritation, midfacial or lower facial spasm, blow spasm and eyelid tic. Injection of tiny doses of botulinum toxin at several sites above and below the eyes can significantly relieve these symptoms. |

| What is Juvéderm™? |
|

| Juvéderm™ filler will help smooth moderate to severe facial wrinkles and folds. Most patients need one treatment to achieve optimal wrinkle smoothing, and the results last about six to twelve months. |

| What are possible side effects? |
|
| Most side effects are mild or moderate in nature, and the duration is short lasting (7 days or less). The most common side effects include, but are not limited to, temporary injection-site reactions such as: redness, pain/tenderness, firmness, swelling, lumps/bumps, bruising, itching, and discoloration. As with all skin injection procedures, there is a risk of infection. |

| Do the injections hurt? |
|
| Injections may cause some discomfort during and after the injection. Juvéderm™ filler is injected directly into the skin using a fine needle to reduce injection discomfort. Physicians may choose to numb (anesthetize) the treatment area to further minimize discomfort. |

| What should I expect following the procedure? |
|
| Your physician will tell you what to expect following treatment with Juvéderm™ filler. Within the first 24 hours, you should avoid strenuous exercise, extensive sun or heat exposure, and alcoholic beverages. Exposure to any of the above may cause temporary redness, swelling, and/or itching at the injection sites. If there is swelling, you may need to place an ice pack over the swollen area. You should ask your physician when makeup may be applied after your treatment. |

| Does the correction last forever? |
|
| No. Correction is temporary; therefore, touch-up injections as well as repeat injections are usually needed to maintain optimal correction. |

What is LATISSE™?
LATISSE™ solution is indicated to treat hypotrichosis of the eyelashes by increasing their growth, including length, thickness and darkness. Eyelash hypotrichosis is another name for having inadequate or not enough eyelashes.
How does LATISSE™ work?
LATISSE™ is believed to affect the growth phase of the eyelash hair cycle in 2 ways: first, it increases the length of this phase; and second, it increases the number of hairs in this growth phase.
How soon will I see results?
The onset of effect with LATISSE™ is gradual. In the clinical trial, the majority of LATISSE™ users saw a significant improvement by 2 months.
How is LATISSE™ different?

| Restylane® and Perlane® |
What are Restylane® and Perlane®?
The Restylane® family of products includes Restylane® and Perlane®. Both are cosmetic dermal fillers that replace lost volume and restore youthful contours to the skin to smooth away moderate to severe facial wrinkles and folds, such as the lines from your nose to the corners of your mouth.
How do I choose between Restylane® and Perlane®?
You don’t have to. In clinical trials, Restylane® and Perlane® were used in the same patients. The hyaluronic acid in both products is made by the same biotechnical process. The difference is in the gel particle size. Perlane® gel particles are larger and designed for deeper injection than Restylane®. Your doctor will decide which product is right for you.
How long do Restylane® products last?
Restylane® products are proven to deliver long-lasting results. Restylane® lasts about 6 months, while some Perlane® patients maintained results for up to 9 months.
Do the injections hurt?
Restylane® products are injected directly into the skin in small amounts by an ultrafine needle. As with any injection, some patients may experience discomfort. To increase your comfort, your doctor will assess your need for pain management and determine what technique is right for you.
