Important Information Regarding Insurance Network Participation for Illinois Eye Center Providers
We are writing to inform you of upcoming changes to the insurance networks in which Illinois Eye Center participates. Effective January 1, 2026, Illinois Eye Center providers may no longer be part of the following insurance networks or there may be temporary interruption in network status:
- Cigna
- Multiplan/PHCS/Claritev
- HealthLink
- Carle Health Physician Partners
- HFN/Zelis
These changes are due to the dissolution of the Carle Health Physician Partners (CHPP) network, with which we currently participate through a group contract. The CHPP group contract also included the Cigna, Multiplan/PHCS/Claritev, and HealthLink networks for Illinois Eye Center. The CHPP network contract will dissolve on December 31, 2025, and will no longer be an active network in 2026.
What This Means for You:
We are actively working to secure direct contracts with the individual insurance networks for Cigna, Multiplan/PHCS/Claritev, and HealthLink. However, as of now, no agreements have been finalized. We will continue to update our website at www.illinoiseyecenter.com with the latest information regarding these networks.
If a contract is not finalized by 12/31/2025 and you receive services after this date, your insurance may not allow benefits if an in-network provider is required for your plan. Your insurance may process your claims as out of network until a contract is finalized to include Illinois Eye Center providers as in-network.
Additional Information:
- If your insurance plan is currently utilizing the Carle Health Physician Partners (formerly also known as UnityPoint Health PLUS) network, please verify with your insurance for new network information for 2026.
- We encourage you to confirm that your Illinois Eye Center provider is in-network before receiving any services.
- Please note, the HFN/Zelis network termination effective December 31, 2025 is final and this network is not currently in discussion for 2026 participation.
